Heart Health

Heart Health & Self-Measured Blood Pressure

MAGZ supports community education focused on blood pressure awareness, hypertension prevention, cardiovascular wellness, and the importance of understanding personal health numbers.

Self-measured blood pressure education can help individuals learn how to monitor blood pressure appropriately, understand their readings, recognize the importance of follow-up, and communicate more effectively with healthcare professionals.

SMBP Program Champion MAGZ Health Services participates as a Self-Measured Blood Pressure Program Champion, supporting increased awareness and access to SMBP information and resources.

Healthy Aging & Dementia

MAGZ supports older adults, caregivers, families, professionals, and communities through education related to healthy aging, dementia, caregiving, chronic conditions, wellness, safety, and maintaining quality of life.

Our dementia education helps strengthen understanding of Alzheimer's disease and related dementias, communication, person-centered approaches, caregiver needs, and available resources.

Resource Navigation & Community Support

Health needs are often connected to needs outside of the healthcare setting. Individuals and caregivers may need help identifying reliable information, community services, aging resources, food assistance, transportation, caregiver support, healthcare resources, or other available programs.

MAGZ may provide educational information or help individuals identify appropriate community resources based on current needs and available services.
